Do I Need Orthotics

For many with foot pain they wonder if orthotics may be the solution for their pain. In some cases, the answer will be yes, but for most the answer is probably no. A second question then asked is custom vs over the counter. Let’s discuss some of the reasons for orthotics.

One main reason is foot deformity or complete loss of foot arch (with associated pain). In such cases, a custom orthotic is probably the way to go. The purpose or value of a custom insert in these cases is to “put” the foot in a more optimal position to function. This will hopefully result in less foot pain AND reduce the risk of other lower body joints complaining of pain as well.

Another reason is significant deviation or surgery on the big toe.  Often the deviation or following surgery will result in reduced motion of that toe. This has a negative impact on walking mechanics, which can result in pain elsewhere. Thus, in cases like these, a custom orthotic accommodating that big toe issue can be meaningful.

Other common reasons a person may consider orthotics include plantar fasciitis, Achilles tendon issues, and posterior tibial tendon dysfunction. With each of these conditions, an off-the-shelf orthotic may be a useful tool in the healing/rehab process. Understand it is unlikely to solve any problem in isolation.

Regardless of the foot or lower body issues, some amount of rehab should be completed. An orthotic is not a magic device. It can be a help but will not fully solve any issue. For greater success with an orthotic, rehab is necessary. This rehab will include: strengthening, joint mobility work, and possibly stretching. And if you are going to spend several hundred dollars on a custom orthotic, you might as well do all you can to make sure to eliminate the pain.
