Burning thumb pain is a common occupational issue. Individuals who work with their hands all day from manual laborers to people pounding away on a computer keyboard are at risk. Regardless of occupation, the question ALWAYS is – what can be done to get rid of the pain?
In the case here, the person’s main complaint is burning pain. Now burning pain tends to indicate some irritation of nervy tissue. This could be the nerve itself or the surrounding tissue which can impact the freedom of movement of the nerve.
See nerves need three things: bloodflow, space, and movement. If any one of these factors is altered, some nerve irritation response can appear. This could be numbness, tingling, or even burning sensations.
Now, in the case of the thumb we have 2 main nerve roots and 2 peripheral nerve branches that could be an issue. As such, each root and branch must be tracked to learn where the issue is happening. Is it at the thumb (local) or referred from the elbow, shoulder, or even the neck? In order to learn this a thorough and systematic examination must be performed assessing each of the nerve roots and branches. This assessment should include reflexes, sensation testing, as well as a strength test. Additionally, a detailed range of motion assessment from the neck to the thumb will also be performed. The information obtain via these 3 key elements and range of motion assessment will help determine the source and what exactly can be done.
Regardless of the exact location, a few key elements will be included in a successful treatment plan. To begin with, the provider will address any motion limitation or stiffness to make sure the joints of the neck/arm are not impacting nerve movement. Next, direct nerve treatments are likely to be implemented. These specific nerve glides have been shown to be helpful in calming irritated nerves and promoting blood flow (a key element for nerve health). Finally, the individual patient presentation will direct and focus other aspects of care as necessary…because in the end it will be about adjusting any program to the needs of the individual with thumb pain.
